This programmer worked fine, using eeproms 24c16 (Nokia 5xxx), 24c64/65 (Nokia 6xxx). And it works with all 24cXX, I guess.
The picture number 1 is that what I use, it doesn't support in-circuit programming.
The picture number 2 is that what I also use, it does support in-circuit programming (just added 9V battery).

The connector in the picture isn't the right one (it is for 14 pins?), but just like the 8 pins one. (U can order it from

Software
I have used separate of progs, but best (this far) is PonyProgrammer. U can download it from here (free ofcourse).
First try to calibrate your eeprom programmer (eeprom should be connected to the eeprom programmer). If failed try again.
If still failed check once more your programmer.
If ok. Try to read your eeprom (set up the eeprom type settings first (the programm won't found your programmer in the setup)). If succesfull your programmer works correctly. If failed and u have self made connector to eeprom test many times (I didn't got it work at first time, u have to keep it very carefully that it wont move during the reading).
If got it succesfull, don't try to write it back, because it won't work because u have to add an external power supply first (eeprom programmer picture 2), or desolver the eeprom off from the phone (It very difficult to solver back - believe me ;-), or just lift up few pins (I don't know which pins).
